Physical Therapist — Telehealth Evaluator & Growth/Ownership Track

About the Role Suru provides outpatient physical therapy in patients’ homes. This role combines Medicare telehealth evaluations with the opportunity to help build and grow the business. You will conduct telehealth physical therapy evaluations and reassessments, create plans of care, and coordinate the transition to appropriate in-home follow-up therapy. You will also learn how to help operate the company through marketing, referral development, clinician recruiting, and process improvement. Eligibility, vesting, grant size, valuation, and performance requirements will be defined separately. Responsibilities Conduct Medicare telehealth physical therapy evaluations and reassessments. Create clear, clinically appropriate plans of care and document findings promptly. Coordinate with patients, families, and treating PTAs or other care-team members. Support referral development, marketing, clinician recruiting, and business operations. Help improve Suru’s clinical, recruiting, and patient-growth workflows. Requirements Active California Physical Therapist license. Active Medicare PTAN is required. Medicare enrollment and telehealth eligibility appropriate for the services performed. Strong clinical documentation and communication skills. Interest in learning business operations, marketing, and recruiting. Compensation This is a contract opportunity. Clinical and business-growth compensation will be discussed based on the final role structure. Equity participation, if offered, will be governed by a separate written plan and is not guaranteed by this posting.
